Making a referral

From first contact to intake review

01

Submit the referral

Send referral paperwork through our contact team, or call with questions before you begin. We accept referrals for individuals across all of North Carolina.

02

Provide documentation

Share the required information so we can understand the person's needs, daily living support requirements, and medication management assistance needs.

03

Intake review

Our team reviews the referral against our residential group home care and 24/7 supervision and support, then confirms whether we are the right fit.

04

Individualized plan

If we move forward, we review the individualized care plan together so the transition is planned around the person's routines and goals.

What to expect

Preparing for a smooth transition

Clear expectations for home visits, transition planning, and the first days in residential care.

Initial home visit

We can arrange a visit so the individual and family can see the shared home spaces, meet the team, and ask questions about daily routines.

Transition planning

Dedicated coordination helps align the move with community integration and activities, medical follow-up, and the individual's personal preferences.

Settling in

Staff provide 24/7 supervision and support while daily living skills and individualized care plans guide each person's first weeks at the home.
